Appearance Reset Request & Order

To request a continuance by mail, before the appearance date, complete the above form by entering your name and the Citation number. Sign the form and file it with the Court.

If your request is granted, the Court will enter the rescheduled date, sign the Order, and return a copy to you. If you do not receive a copy of the approved reset, you should check with the Court because if the request is not granted, you must be present on the date and at the time originally scheduled.

Driving Safety Course Request & Order

To handle deferment for driving safety course for a single moving violation by mail, before the appearance date, the Defendant must complete the Driving Safety Course Request and Order form (PDF) by entering their name, the Citation Number, their signature, their phone number, and their mailing address or fax number.

The Deferment Fee must be paid in full with the submission of the Request and Order Deferring Further Proceedings. The Defendant must submit copies of their driver's license and current liability insurance card with the Request. The Court will enter the Cause Number, the last day of the deferment period, and the date of the Compliance Hearing. The Court will date and sign the Order, and return a copy to the Defendant via mail or fax as directed.

Deferred Disposition Request & Order

To obtain Deferred Disposition for a single moving violation by mail, before the appearance date, the Defendant must complete the Deferred Disposition Request and Order form (PDF) by entering their name, the Citation Number, the amount of the deferment fee, their signature, their phone number, and their mailing address or fax number where indicated. The amount of the Deferment Fee shall be the amount of the standard fine as shown on the Citation or Table of Offenses plus $25.

To handle deferment by mail, the Deferment Fee must be paid in full with the submission of the Order Deferring Further Proceedings. The Court will enter the Cause Number, the last day of the deferment period, and the date of the Compliance Hearing. The Court will date and sign the Order, and return a copy to the Defendant via mail or fax as directed.
